Church Connect Form Card

Encouraging your people to fill out Form Cards in Church Connect will cut down on the workload for your church leadership and save money on printing costs!

Creating a Form Card

After creating a new Form Card (or by selecting a Form Card you've already created), designate the ChurchTrac user who will receive emails containing the responses from each form submission.

Next, select "Form Questions" button and begin adding your questions.

Proceed in creating your first field. You can select from multiple field types and even add a description. Continue adding more fields. Once you've finished, you can customize the form with field requirements, visibility, and order.

Templates

With the new customization features available in the site setup, you can also create a Form Card Header (or Hero) and Body section(s). You can choose from a curated list of templates, or create your own with custom fonts, text, and background options.

Step 1: Edit Page Content

First, choose the Form Card you want to customize. Then, select the "Edit Page Content" button to open the card editor.

Step 2: Add a Hero or Body Section

Next, you can select either the Header (or Hero) or a Body Section(s) of the card to edit. Select "Add Content" to choose from the available templates or create your own custom design.

Form Card Responses

To view your Form Card responses at once, select the "Form Responses" tab on your Church Connect Screen. You can either view all responses to all forms or select a form on the left and view responses to only that form. Form card responses can also be printed out or exported to CSV, which allows you to store the data elsewhere.

Embedding a Form Card

You can embed a Form Card directly onto your church’s website, allowing visitors to fill out forms without leaving your site.

To do this, go to Church Connect and select the Form Card you want to embed. Select Show Additional Options, then scroll down to the Form/Giving Embed Domain section.

Enter your website’s domain and save your changes. This will generate an iframe code that you can copy and paste into your website’s HTML.

You can embed a Form Card directly onto your website so visitors can fill out forms without leaving your site.

Share a card's URL

If you want to share a card's URL, simply login to your Church Connect account and select the card. Once you've opened the card, copy and paste the URL from your browser's toolbar.

Use a form card for Twilio verification

Twilio requires that you verify your toll-free messaging number. Churches must follow the same texting guidelines as businesses. Per carrier guidelines, all businesses are required to provide proof of how they are collecting their consumer's consent. An online form card is the best way to do this for churches. Learn more ›

Use Twilio auto-replies

If you have a Twilio account integrated with your ChurchTrac account, you can set up auto-replies so that your people can text a keyword to your church and receive a text in response. Link your form cards in these texts to make it easier for everyone to navigate to them.

Remove fields for members

You can set a form field to show or not show to the user based on whether or not that person is logged in. When logged in, we'll automatically inject the user's Name and Email into the form submission for you, so you only need to ask for this information when the member has NOT logged in.
